Weird Issues After Re-Spawn

I'm sure everyone has had this happen a few times now. After you die there can be odd things upon spawning again. I've spawned with no ammo...forcing a re-load at spawn. The other more strange bug I've had as an assassin is that my dash button after re-spawn would not activate or recharge. I literally had to die again before it would work.
