I have been penalised after my opponent didn't accept the ranked match and I clicked "decline" after the screen remained showing "0 seconds" for a while. There are some bugs like this one with matchmaking I simply think it needs more polishing (for example there are a lot of brief UI/menu glitches when a match is found or after a match ends).
