This glitch has happened since day one on XBox. What happens is the game just looses track of you. Nobody knows how, or how to fix it. All we know is you die, usually in some big way like grapples or juggled by multiple teammates. For some reason, sometimes when you get ultra-killed, your health remains at 0 and you stay alive, bound in one place able to shoot. Sometimes, this is really highly exploited. An assassin juices in a base, gets her *** handed to her, but this happens to her, then she is stuck in the enemy base, able to wipe turrets with shuriken launcher all game. The honorable thing is to back out and come back in, or stand there the rest of the game without exploiting it. It isn't controllable, so it can't be used relably to cheat or anything. If you DO find some way to consistently reproduce the glitch, please report it to Uber, they might reward you.
