This happened to me a few times, and it has been happening a little more frequently now. It is not a big deal because it happens maybe 1 out of 10 games, but it still gets a little annoying. I will hack my firebase, and my skill will go down and need to recharge, but my firebase will not be hacked. Next one has happened a lot to me as assassin, but when I die and re spawn sometimes, I can't dash. My dash skill makes a noise as if to say "access denied", and I also can't use lunge. This does not correct itself until I die. Last one, and this is not technically a glitch, but when there is a host migration, whatever firebase you had down will be gone. Your skills are at zero and need to recharge. I have only experienced this with assassin ans support because that is what I play most so I am not sure if the other classes have this issue.
