What is it's purpose? Why is it there? Does she literally have an eye on her forehead that requires this? If she does, why can't I see the end of the match right at the beginning? Or why doesn't it have the specific purpose of seeing cloaked assassins from further away than other classes?

It seems like an odd aestetic design choice without a purpose? A third eye would give you vision of what would happen in the future and past of our linear time line. It's positioned slightly lower than where the human third eye is. Is there a purpose, inside joke, or trying to be slightly original?

That had a purpose. Sam had 3 vision modes. Heat, electric, and zoom or xray, depending on the game.

It so obviously is. It's a definate copy of the third eye that all humans have. She must be the next step in evolution for the human race. Someone that's been genetically altered to unlock her true human potential.

This image even suggests she has a deformity on her face, like she really does have a third eye.

That's a tough question. Depending on how messed up she looks like. A paper bag only goes so far. :p

Be pretty funny if Uber added a "maskless" Assassin gear item but it puts a paper bag on her head. (wouldn't really fit the attitude/artstyle though but it'd be a funny community trolling.)

Say, has anyone here played Metroid Fusion? I'd like to re-introduce you to a little someone called "Nightmare"
