So been trying all the possible fixes on the provided wiki for support, and now i am here with the dxdiag, and the PA crash. I hope some one can help me make heads or tales of this.
Start by updating your out of date GPU driver from 2016 with latest 390.65: http://www.nvidia.com/download/driverResults.aspx/129079/en-us https://www.reddit.com/r/nvidia/comments/7oylg2/driver_39065_faqdiscussion_thread/ 372.90 was a rough release with all the power management changes. I'm surprised you haven't had problems with other games. Many of your other drivers also look pretty old from 2014. I would also expect a newer BIOS with the AMD FX-8350 / GTX 960 combo. Probably F13. What motherboard model is that?
Motherboard is a GIGABYTE 970A-DS3P Am3+ sockets. SMBIOS version is 2.7 from 2014. as for the video driver, no other games have caused issues. and the sad thing is my computer has always said its up to date... Manually updated my graphics and Ethernet drivers so far, cant find to much else, and not sure what else needs it
PA will push your hardware to the max especially with a local server. I recommend you take a look at the downloads on the Gigabyte site for your motherboard revision which is 3.0 based on your current FB BIOS: https://www.gigabyte.com/Motherboard/GA-970A-D3-rev-30#dl (select Windows 8.1 64 bit) Windows may have newer drivers already or at least Windows 10 would. Upgrading the BIOS is probably not necessary unless you want to use faster RAM or an upgraded processor. FB BIOS is fine for a FX-8350. I have tried PAT again since i updated the other 2 drivers alas no luck there. Also if possible i would prefer to skip the bios update as i would rather not mess with that, and mess up my computer. EDIT: added in new crash logs after updates etc.

The logs and dxdiag don't show any PA crashes. There are crashes for windows update, stellaris, tera, twtich, etc The dxdiag does show LiveKernel hardware failure events. Try the clean boot.
What is happening is the game will open, full screen then go to window itself, i have windowed on in options since i read that was an issue but it still starts in full most of the time and switches. stays on menu for anywhere from 2-30 seconds and just crashes straight to reboot. Even if i dont touch anything computer wise after launching it it still crashes. I will update those 3 didnt know my headset and mouse would already be out x.x, just got them last summer lol. Also Mikeyh I have done a Clean Boot, and i did it step by step as what was on the Wiki(EDIT from forums).
Updated those 3, even did another clean boot after updates, still crashing with in 30 seconds of launching the game.
Those LiveKernel hardware failure events and system level crash reboot look like a hardware / drive issue that would be caused by bad / incompatible RAM, overclocking, etc. What does the Windows Event Viewer show before those crashes? Do you have any other OpenGL games?
The only other OpenGL i can think of right away is counterstrike global offensive. Used to have issues with that due to a corrupt line of ram, but replaced that a while back and havent had issues since. well may be my PSU whats the best way to test it? ive ran mem test already no issues there (8 hours over night) and as for heating ive ran speccy for a long while to keep tabs and its stayed below the threshhold everytime
